چکیده

OBJECTIVES Discordance between the Gleason score (GS) on needle biopsy (NB) and the GS of the radical prostatectomy (RP) specimen is a common finding. The objective of this study was to evaluate the prognostic significance of these discrepancies with respect to outcomes following RP. METHODS In the study, 6625 men treated by RP were categorized as having NB=RP (68.8%), NBRP (6.2%) GS, and stratified for analyses into RP GS groups. The Kaplan-Meier method was used to analyze differences in biochemical recurrence-free survival (BRFS), and multivariate Cox analyses were performed to estimate the independent relative risk of progression associated with GS discrepancies. RESULTS Across multiple RP GS strata (3+4, 7, 8, 8-10), patients with a lower NB GS experienced significantly better BRFS than patients with equal NB and RP GS (all p<0.05). NBRP GS had poorer BRFS than patients with NB=RP GS across multiple RP GS strata (< or =3+3, 3+4, 7; all p<0.05). NB>RP GS was independently associated with worse (pooled HR, 1.91, p<0.001) BRFS probabilities, within and across RP GS strata. CONCLUSIONS Our data suggest that the GS of the NB adds additional prognostic value to the RP GS in a consistent manner that may be applicable to strategies of risk stratification and patient counseling after surgery.

Correlation between Gleason scores in needle biopsy and corresponding radical prostatectomy specimens. A twelve-year review.

Background: Presence of discordance between the Gleason score on needle biopsy and the score of radical prostatectomy specimen is common and universal. In this study, we determined the accuracy of Gleason grading of biopsies in predicting histological grading of radical prostatectomy specimens and the degree of overgrading and undergrading of prostatic adenocarcinoma i...

Exploring the Effect of PSA Density and Prostate Size on Concordance of TRUS Biopsy Result with Radical Prostatectomy

Background and Objective: Today’s, the Gleason grading system is well known as the world’s most commonly used histological system for prostate cancer. It provides significant information about the prognosis. This prospective paper assessed the correlation of transrectal ultrasound (TRUS) guided biopsy and radical prostatectomy specimens in terms of Gleason scores. In this matter, the effect of ...

بررسی مقایسه‌ای بین نمره گلیسون بیوپسی‌های پروستات و نمونه‌های رادیکال پروستاتکتومی

The present study was undertaken to compare the gleason score and histologic grade of needle biopsy specimens to those of radical prostatectomy(RP) specimen to examine how accurately biopsy specimens reflect the corresponding excised specimens. In this comparative-descriptive study, gleason scoring was made on the needle biopsy specimens from 57 patients with prostate cancer and then ...

[Concordance of Gleason histological scoring for prostatic cancer in needle biopsies and the surgical piece obtained during radical prostatectomy].

BACKGROUND The Gleason histological score is an independent prognostic factor in prostate cancer that helps in therapeutic decisions. AIM To analyze the concordance between the Gleason score of needle prostatic biopsy and the score from the study of the surgical piece obtained during radical prostatectomy. MATERIAL AND METHODS Retrospective analysis of 212 cases of prostate cancer, diagnose...

DNA-cytometric grading of prostate cancer systematic review with descriptive data analysis

Gleason-score <=6, assessed on core needle biopsies, is an essential prognostic parameter to offer the strategy of Active Surveillance (AS) to patients with locally confined cancers of the prostate. Yet, its interobserver reproducibility is low (48-70%) and its prognostic validity unsatisfactory. An option to complementary assess the malignant potential of these cancers are objective DNA-ploidy...
